Conversion of modern notation to neumes

Hi,  
I'm working on creating an illuminated manuscript, using a chant-based piece I wrote in college. However, I wrote it in modern notation, and I'd like to convert the modern notation to neumes, complete with four staves and the correct clef.  
 
Is there someone who could help with this?
